KSHSAA Sports Physical Evaluation Form

Any junior high or high school athlete planning on participating in sports for the 2026-2027 school year must have the KSHSAA Pre-Participation Physical Evaluation completed and turned in to the office before they will be allowed to start practice in August. This can be done now and count for the 2026-2027 school year. The physical can be completed by your family physician or at the Cunningham Clinic. Please have your portion of the physical form filled out (including parental signature) BEFORE the appointment.

PLEASE NOTE: The Cunningham Clinic will not be available to complete sports physicals from Monday, August 3 through Friday, August 7 due to scheduled vacation time. The first day of fall sports practices is Monday, August 17th.
